We help individuals and families understand their options, access the right services, and manage care over time. We take a personalized approach to every situation. Whether you’re navigating a new diagnosis, managing a chronic condition, or planning for long-term care, our team works alongside you so you feel confident and supported every step of the way.

What we do

Making sure every piece of your care works together

Care management is about more than coordination—it’s about making sure every piece of your care works together effectively.

Our team assesses your needs, connects you with the right services, and provides ongoing support as those needs evolve. We help you understand your options, communicate with providers, and make informed decisions about your care. Through education, advocacy, and proactive support, we help reduce confusion and prevent gaps in care—ultimately decreasing hospitalizations.

Specialized support across the community

We provide clear, unbiased information so you can make confident decisions about your healthcare and financial planning.

Care Management

Our registered nurse care managers provide health education and support in coordination with physician organizations—helping manage chronic conditions, improve outcomes, and reduce unnecessary hospitalizations.

Referrals: RN Care Management requests can come from physicians who are part of PMC, anyone participating in the care of a qualifying patient, or self-referrals.

Waiver Support Services

For individuals enrolled in waiver programs, we offer additional support to keep services effective and aligned with their needs—especially after a hospitalization, incident, or injury.

Referrals: Referral from the Waiver Support Coordinator assigned to the participant.

MI Options Counseling

Choosing the right long-term care option can be difficult. Our person-centered MI Options Counseling helps individuals and families understand and access available resources and create a plan.

Referrals: Contact a MI Options Counselor directly through the statewide 2-1-1 call center, or through internal referrals.

SHIP/MIPPA Counseling

Understanding Medicare and Medicaid coverage can be confusing. Our SHIP/MIPPA counseling helps individuals navigate benefits, explore coverage options, and get answers to important questions.

Referrals: Direct referrals or through the 2-1-1 call center.

Does insurance cover this service?

There is no cost associated with MI Options Counseling, and insurance covers services provided by RN Care Management and Waiver Support Services. There are no out-of-pocket charges.

How do I know if I qualify?

There are many options available to provide the services you need within your budget. Give us a call and we’ll help determine if you qualify for services.

How quickly can services start?

Our Care Management services start within 3 days or less of your call, or when a referral is made from an appropriate party. Once we determine your needs and assess your options, we’ll begin connecting you with the programs and services available to you.
