In the past, nursing homes were often the only option for older or disabled individuals who needed help caring for themselves. Today, there are more options that allow you to live independently while receiving nursing-facility-level care in your home. The MI Choice Waiver Program is one program you may be eligible for.
The MI Choice Waiver Program is a Michigan Medicaid program designed to help eligible adults receive care at home instead of in a nursing facility. It provides access to a wide range of services that support independence, safety, and quality of life.
The dedicated Waiver Division at A&D Home Health Care works closely with participants and their families to coordinate the services needed to make this possible. Through ongoing assessment, care coordination, and proactive support, we ensure each person and family is supported, respected, and empowered to live safely at home.
Eligibility
Who is eligible for the MI Choice Waiver Program?
Our Intake Department can take your call and perform a short phone assessment to determine whether your case can be assigned to an admit team consisting of a Registered Nurse and Social Worker. If you don’t meet the MI Choice Intake Guide (the MIG), our team will provide additional information and community resources to help you stay at home. If you pass the MIG, a team can assess you for eligibility in your home, a hospital, or a nursing home.
In general, eligibility requirements include:
- Being eligible for Medicaid in the State of Michigan
- Meeting Nursing Facility Level of Care criteria (NFLOC)
- Gross monthly income not exceeding $2,901.00 per month (including SSI, RSDI, Pension, Workers’ Compensation, and Railroad Retirement)
- Total individual assets less than $2,000.00 (including checking, savings, CDs, savings bonds, trust funds, or life insurance with specific face value)—this does not include 1 house and 1 vehicle

Our role goes far beyond simply arranging services. We assess each participant’s unique needs, coordinate care across multiple providers, and continuously monitor and adjust services to ensure they remain effective.
We also take a proactive approach to risk management—helping prevent complications, avoid unnecessary hospital visits, and support long-term stability. This ongoing involvement means participants experience greater safety, improved quality of life, and a stronger voice in their care.

How quickly can services start?
Services typically begin after eligibility is confirmed and the initial assessment and service plan are completed. Some services can start quickly, while others may take additional time depending on provider availability and participant needs. Our goal is to put supports in place as soon as possible while ensuring they are appropriate and safe.
Do I need a doctor's referral to receive services?
A formal referral is not always required to start the process, but medical information may be needed to determine eligibility. Participants must meet Nursing Facility Level of Care (NFLOC) criteria, which includes a clinical assessment of their needs.
Does insurance or Medicaid cover these services?
MI Choice Waiver services are covered through Medicaid for individuals who meet both financial and functional eligibility requirements. Our team can help guide you through the eligibility process and answer questions about coverage.
Are all Michigan residents eligible for the waiver?
The Waiver Division of A&D Home Health Care is one of 20 Waiver programs in Michigan. We serve a number of counties in Mid-Michigan and Michigan’s Thumb and Saginaw Valley area. If you’re not in this area, you may still be eligible—and we can refer you to an appropriate Waiver Program.
